I complete therapy as a cash based/out of network provider. I can provide receipt of services for you to submit to insurance providers (superbill). Most insurance companies reimburse occupational therapy services although you will want to confirm out of network benefits. You can submit your provided receipt directly to your insurance. Please be aware that if you plan to submit the claim your insurer may require a medical doctor referral for care.
Often cash based care is cheaper than insurance based care!
As a cash based provider I cannot provide care to those who have Medicare. I am happy to assist you with locating a provider who does!

Home/Office Based Evaluation
$225 Home-$200 Office
Your evaluation will be up to 90 minutes in length. I bring all needed supplies, including a treatment table for home based visits. Following your session, I will email a summary of your evaluation, including your treatment plan and videos of activities/exercises we covered. I will follow up 1 week post via email to continue addressing your needs.

Telehealth Evaluation
$100.00
Telehealth Evaluations are available to Ohio residents. All telehealth appointments are conducted via Jane App, an online health record system, which is HIPPA compliant. Your evaluation will be 60 minutes in length. Following our session I will email you a summary, including videos, of all we covered. I will email you1 week after your evaluation to answer questions and continue our work towards your goals.
Home/Office Based Treatment Session
$150
Your treatment will be 60 minutes in length. I will bring all the needed supplies, including a treatment table to your home based session. Following our session I will email a summary and videos of what we covered to assist you with implementing your plan.

Telehealth Treatment
$50.00
Telehealth treatment sessions are available to Ohio residents. All sessions are conducted via Jane App, which is HIPPA compliant. Your session will be 30 minutes in length. I will email a summary and videos of what we covered to assist you with implementing your plan.
I offer services for those experiencing financial hardship for a reduced rate of $50.00 for care. I have a financial hardship policy and application that are required to receive reduced rate care. I am able to provide services for a reduced rate to one eligible client per month. Please inquire directly to obtain the policy and application.
What to expect
Prior to meeting all intake paperwork will be emailed.
The first time we meet we will conduct a thorough evaluation which can range from 1-1.5 hours in length. We will cover your history, concerns and functional goals! I listen to your story and needs!
Your evaluation will include extensive education.
Your evaluation may include external and potentially internal review of the pelvic floor and body. Pending the results we will make a holistic treatment plan to return you to your best life. We will work together throughout to ensure that all your needs are met!
We may utilize biofeedback, activity modification, manual therapy including myofascial release or visceral fascia mobilization, training to downregulate the nervous system, therapeutic exercises, + dietary modifications.

Covid -19 Protocols:
I will continue to be masked at all in person visits
I will be cleaning all equipment and the portable table between each visit
I will be monitoring myself for symptoms (including cough, shortness of breath or difficulty breathing, fever, chills, muscle pain, headache, sore throat, loss of taste or smell ). I ask that you additionally notify me of symptoms of yourself or family.
I will monitor my temperature and cancel appointments if above 100.4 . I ask you to additionally cancel if you have a fever.
I will remove my shoes before entering your home.
I will wash hands upon arrival and before leaving.
